FAIR CREDIT REPORTING ACT (FCRA)

In connection with my application for employment, independent contractor engagement, technician position, or other work opportunity with Yes Group LLC (“Yes Group”), I understand that Yes Group may obtain a consumer report about me for employment or contractor screening purposes, as permitted by the federal Fair Credit Reporting Act (“FCRA”), 15 U.S.C. § 1681 et seq., and applicable state and local laws.

I understand that the consumer report may include information regarding my:

• Identity and Social Security number verification
• Criminal history
• Motor vehicle and driving record
• Employment history
• Education and professional credentials, when applicable
• Other background information legally permitted and reasonably related to the position or services for which I am being considered

I understand that Yes Group may use a third-party consumer reporting agency to obtain this information.

AUTHORIZATION

By signing below, I voluntarily authorize Yes Group LLC and its authorized agents or consumer reporting agencies to obtain a consumer report about me for the purpose described above.

I understand that this authorization does not authorize the release of medical information and does not authorize access to my personal financial account information.

I certify that the information I have provided in connection with my application is true and accurate to the best of my knowledge.

I understand that providing this authorization does not guarantee employment, engagement, assignment, or approval as an independent contractor or technician with Yes Group LLC.

I acknowledge that I have read and understand this Consumer Disclosure and Authorization and voluntarily authorize the background screening described above.

By signing above, I acknowledge receipt of this disclosure and authorize Yes Group LLC to obtain the consumer report described herein.
